The Fort Worth Independent School District is accepting RFP's (Request for Proposals) for Supported Employment Transition Services Provider Agent(s). Agents will provide skills training to FWISD Students with moderate to moderately severe cognitive impairments, preparing them for supported employment in a community setting. The goal of this partnership is to affect job skills analyses, targeted career path placement, as well as work skills development, aligned with student’s interests and career pathways. Contracted services will be provided

to eligible FWISD Students completing post-secondary transition skills development in a community-based setting.

The period of performance is from July 1, 2023, or date of execution, whichever is later, through June 30, 2024, with the option to extend for five (5) additional years in one (1) year increments. All purchase orders dated and issued will be subject to the terms and conditions of this RFP.
