The Fort Worth Independent School District is accepting RFQ's (Request for Qualifications) from qualified, licensed physicians to perform Otolaryngologic (ear, nose, and throat) examinations for students suspected of having a hearing loss. The successful contractor will work in conjunction with FWISD staff to review related documents, conduct the required examinations, complete, sign and submit related report(s). FWISD will be responsible for identifying students for the evaluations, obtaining necessary written consents for the examinations, coordinating with office staff, if required, and to set appointments. Appointments will be open to FWISD students and/or to students attending any one of the 7 RDSPD member districts.

The period of performance is from date of award to November 30, 2023 with the option to extend for four (4) additional years in one (1) year increments.

All purchase orders dated and issued will be subject to the terms and conditions of this RFQ.
